瑞芬太尼抑制气管插管应激反应的应用进展

         

摘要

Remifentanil,a relatively new synthetic opioid,is a selective μ opioid receptor agonist, which is metabolized by non-specific esterases present throughout the body and tissue because of its special ester linkage. As a result, the drug has an ultrashort duration of action and metabolism, and the pharmacokinetics is not influenced by hepatic or renal failure. What's more,its metabolism is independent of the duration of infusion. So,remifentanil is widely used in induction and maintenance of anesthesia. Remifentanil has the functions of analgesia and sedation as well as ventila-tory depressant, muscle rigidity, nausea, vomit, hypotention and bradycardia. This paper reviews the effections of remifentanil used in induction to attenuate the responses to laryngoscopy and tracheal intubation.%瑞芬太尼是一种新型的阿片类麻醉药,是高选择性的μ阿片受体激动剂,因其化学结构中有独特的酯键,所以被血浆和组织中广泛存在的非特异酯酶快速水解代谢,药代动力学不受肝肾功能的影响.瑞芬太尼起效迅速,血脑平衡时间较短,代谢快,无蓄积,因此被应用于临床麻醉诱导气管插管和麻醉维持阶段.瑞芬太尼具有镇痛、镇静作用的同时,也有呼吸抑制、骨骼肌强直、恶心呕吐、低血压和心动过缓等不良反应,现对瑞芬太尼应用于抑制气管插管应激反应的效果进行综述.
