目的:研究股骨近端防旋髓内钉与股骨近端锁定钢板治疗高龄股骨转子间不稳定骨折的临床疗效. 方法:选取2012年5月至2014年5月我院收治的高龄股骨转子间不稳定骨折患者100例,根据固定方式不同将患者分为A组( n=55例)和B组( n=45例) ,其中A组给予股骨近端防旋髓内钉治疗,B组给予股骨近端锁定钢板治疗,随访患者1年,应用Harris评分评价1年后患者的关节功能情况,比较两组手术时间、术中出血量、愈合时间以及并发症. 结果:A组Harris评分优良率94.5%(52/55)显著高于B组的75.6%(34/45),两组比较差异具有统计学意义(P<0.05);两组手术时间和术中出血量比较无统计学意义( P>0.05) ,A组愈合时间显著短于B组,两组比较差异具有统计学意义(P<0.05);两组并发症比较无统计学意义(P>0.05). 结论:股骨近端防旋髓内钉治疗高龄股骨转子间不稳定骨折有利于术后愈合,且并发症较少,关节功能恢复好.%Objective:To study the clinical effect of proximal femoral anti rotation intramedullary nail and proximal femoral locking plate in treatment of unstable intertrochanteric fracture in aged patients. Meth-od:100 cases with unstable intertrochanteric fracture in aged patients were selected from our hospital from May 2012 to May 2014, and divided into group A ( n = 55 patients) and group B ( n = 45 patients) ac-cording to different ways of fixing, then group A were given proximal femoral anti-rotation intramedullary nail for treatment, group B were given proximal femoral locking plate for treatment, the patients were followed up for 1 year, joint functionafter were evaluated in Harris rating, then the operation time, blood loss, healing time and complications of two groups were compared. Result:Harris score excellent rate of group A was 94. 5%(52/55),significantly higher than 75.6% (34/45) in group B, the difference was statistically significant(P<0.05);Operative time and blood loss of two groups had no significant difference (P> 0.05), healing time of group A significantly shorter than that of group B, the difference was statistically significant ( P<0. 05);Complication of two groups had no significant difference ( P>0.05) . Conclusion:Proximal femoral nail anti-rotation in treatment of unstable intertrochanteric fracture in aged patient is conducive to healing after surgery, has fewer complications and joint function recovers well.
