    • 摘要: “人民对美好生活的追求就是我们努力的方向”。作为创伤骨科医生,我们肩负的使命就是救死扶伤,应用自己的智慧和劳动精心治疗肢体伤残者,使他们尽快恢复活动的功能,重新获得生活和劳动的能力,回归社会重返建设岗位。问题是,肢体损伤的情况随时可能发生,病情纷繁复杂,使身居临床一线的医生面临严峻的挑战。有因损伤部位带来的困境,有因特殊的损伤类型存在处理的陷阱,有因损伤特征易被忽视而造成不良后果,更有甚者,虽有多种处理方法而临床却无从选择,不一而足。

    • 摘要: 目的 分析预见性创伤护理对急诊创伤性骨折患者救治效果的影响.方法 用2018年3月-2019年3月院内急诊收治的72例创伤性骨折患者,按照随机分组方式均分两组,各36例.参照组进行常规护理,观察组用预见性创伤护理,对比相关情况、并发症.结果 护理后,观察组相关情况好于参照组;观察组并发症少于参照组,差异有统计学意义(P<0.05).结论 预见性创伤护理可协助急诊对创伤性骨折患者病情做具体分析,护理意识和护理措施都较超前,急救过程更顺利,护理影响力大.

    • 摘要: 体力劳动或体育锻炼时,如果动作不当可引起膝部扭伤、手腕关节脱臼、手指挫伤等。日本《现代农业》杂志,介绍了一些快速治疗这些损伤的方法,可能对我们有一定启发,现分别介绍于下。膝部积水膝部扭伤,可引起局部疼痛、积水和肿胀,如立即施以快速疗法,效果显著。即使膝部积水量很多,用这种方法治疗也能很快消肿;特别严重的病例则需要进行2~3次治疗方能见效。

    • 摘要: 目的 探讨损伤控制性外科理念在断肢再植中应用的临床疗效.方法 2013年6月-2015年3月,收治9例肢体离断合并失血性休克的患者,均采用损伤控制性外科技术(简单手术→复苏→断肢再植手术).结果 9例患者生命均成功挽救,离断肢体再植均获成活,创面均Ⅰ期愈合.所有病例术后随访12~18个月,平均(14.7±1.5)个月,肢体外形好,运动可.Tamai评分为49~94分,其中优5例,良3例,可1例,优良率88.9%.结论 采用损伤控制性外科技术,不仅能更好地抢救生命,还能更好地再植肢体,损伤控制性外科理念在断肢再植中的应用是正确的,值得推广.%Objective To investigate the effect of damage control surgery on replantation of severed limbs. Methods From June 2013 to March 2015, 9 patients with acute hemorrhagic shock of severed limbs were admitted to our center. All patients were treated by damage control surgery. Results 9 patients survied and the severed limbs in all cases were replanted successfully and achieved primary healing of wound after operation. 9 patients were successfully followed up for 12 to 18 months, with an average of (14.7 ±1.5) months, the limbs had satisfactory appearance and motion. According to Tamai's classification, Tamai score increased from 49 to 94 at final follow-up, the results were excellent in 5 cases, good in 3 cases, fair in 1 case, with an excellent and good rate of 88.9%. Conclusion Damage control surgery is a valuable method for treating severed limbs, it is worthy of popularization.

    • 摘要: Limb injury is the common type of trauma in the battlefield,with high disability and mortality.Ultrasound is considered as the prior imaging examination by the features of convenience,easily operated,quick imaging,and so on.Microultrasound can be applied in the battlefronts,assisting the treatment of "golden 1 hour" after limb injury.Ultrasound can be used to determine the location and severity of limb blood vessels injuries,to identify muscle injury and osteofascial compartment syndrome.In addition,it can be applied to diagnose bone fracture,rupture of tendon and ligament,and foreigh bodies of limb.In recent years,nerve block anesthesia and analgesia can be performed under ultrasound guide.All the above is favorable of persistence in combat.As the emergence and clinical application of high-level microultrasound,it is with great value in the treatment of "golden 1 hour" after limb injury.

    • 摘要: 目的:探讨严重肢体挤压伤的院前院内一体化救治方法和效果。方法回顾性分析2008年5月~2015年7月收治的严重肢体挤压伤患者49例的院前院内一体化救治的临床资料,男性44例,女性5例;年龄19~82岁,平均43.7岁。地震伤5例、道路交通伤20例、工矿事故伤17例、其他7例。受伤肢体:肩背上肢8例、骨盆及下肢41例。合并骨盆及肢体骨折脱位32例(闭合伤20例,开放伤12例),多发伤44例。结果院前救援时间35min~49h,现场救援建立静脉通道并输液19例,止血带使用31例。全部病例在解压后行心电监测,救援现场行小腿筋膜室切开1例,膝关节离断1例。院内救治包括清创后负压封闭引流14例,筋膜室切开减压后负压封闭引流17例,负压引流有效率96.8%(30/31),使用负压引流组无截肢者;发生挤压综合征28例(57.1%),其中合并高钾血症11例,肾功能衰竭8例;使用呼吸机辅助治疗8例,连续性肾替代治疗8例。治愈31例;好转13例;未愈5例,包括截肢3例(6.1%),死亡1例(2.0%),死因为高钾血症致心肺复苏失败。结论重视止血带和心电监测在严重肢体挤压伤院前救援中的应用,实施负压封闭引流技术可减少严重肢体挤压伤的截肢率。%Objective To evaluate the therapeutic methods and effect of pre-hospital and in-hospital inte-grated treatment for severe crush injuries of extremities . Methods The clinical data of 49 patients with severe crush injuries of extremities from May 2008 to Jul.2015 in Chongqing Emergency Medical Center was retrospectively studied.There were 44 males and 5 females,with age ranging from 19 to 82 years (average,43.7 years).There were 5 cases of earthquake injury ,20 cases of traffic accident injury ,17 cases of mining accident injury ,and 7 cases injured by other causes .Injured sites of crushes were as follows: shoulder and upper limbs in 8 cases ( 16.3%) , pelvic and lower limbs in 41 cases(83.7%).Thirty-two cases(65.3%) were associated with fractures and disloca-tion of pelvis and joints,and 44 cases were associated with multiple injuries (89.8%).Results Pre-hospital time was 35min-49h.Venous pathway was established and infusion was applied in 19 cases during on-the-scene rescue and tourniquet was applied in 31 cases (63.3%).Decompressive fasciotomy was performed in 1 case and amputa-tion was performed in 1 case during on-the-scene rescue .Electrocardiogram monitoring was performed in all cases after decompression.In-hospital therapeutic measures included vacuum sealing drainage (VSD) in 14 cases after debridement and 17 cases with decompressive fasciotomy . The effective rate of VSD was 96.8%( 30/31 ) and no case was amputated in this group .The incidence rate of crush syndrome (CS) was 57.1%(28/49),of which 11 cases (22.4%) were combined with hyperkalemia ,and 8 cases with renal failure.Eight cases were treated with ventilator assisted therapy and 8 cases were treated with continuous renal replacement therapy ( CRRT ) . At the end,31 cases(63.3%) were cured,13 cases(26.5%) were improved,and 5 cases(10.2%) were uncured,which included 3 cases ( 6.1%) of amputation and 1 death case ( 2.0%) died from failed cardiopulmonary resuscitation due to hyperkalemia .Conclusion Tourniquet and electrocardiogram monitoring should be emphasized in pre-hos-pital treatment of severe crush injuries of extremities .Application of VSD in in-hospital treatment can reduce the in-cidence rate of amputation of severe crush injuries of extremities .

    • 摘要: 目的:探讨V S D负压引流治疗肢体损伤感染创面的护理要点,为二期皮瓣或植皮手术做好准备.方法:2013年12月~2014年12月,对46例应用VSD治疗肢体损伤后创面污染严重或软组织缺损患者的VSD专科护理、疼痛护理、感染护理方面进行观察.结果:46例应用V S D治疗的患者,创面感染控制,肉芽生长良好,二期手术皮瓣或植皮全部成活,无护理并发症发生.结论:密切的护理观察和严格的引流管理,是护理的关键环节,为二期皮瓣或植皮手术创造了有利条件.

    • 摘要: 目的:探讨罂粟碱给药途径对肢体损伤血管修复术后皮肤温度的影响及保护作用。方法选择2008年10月~2010年7月佛山市中医院骨二科的住院患者105例,采用随机数字法将其分为A组(25例)、B组(26例)、C组(27例)和对照组(27例)。A组在术后即刻口服罂粟碱,B组在术后即刻肌注罂粟碱,C组在术后即刻静脉输入罂粟碱,对照组则常规治疗,不运用任何血管扩张药物。观察四组患者皮瓣成活面积、成活率、血管危象发生率及皮温差,及不同用药途径的皮温升高时间和持续时间。探讨肢体损伤血管修复术后皮肤温度的影响与给药途径的关系进行。结果A、B、C组皮瓣成活面积显著高于对照组,差异有统计学意义(P0.05)。结论罂粟碱虽然给药途径不同,肢体损伤血管修复术后患侧的皮肤温度均可得到提高,其中以静脉给药途径最佳,肌内注射次之。%Objective To explore the influence and protective effect of skin temperature after repairing vascular injury in limbs by papaverine administration. Methods 105 hospitalized patients from October 2008 to July 2010 in the Sec-ond Department of Orthopedics in Foshan Hospital of Traditional Chinese Medicine were collected,and they were di-vided into 25 cases of group A, 26 cases of group B, 27 cases of group C and 27 cases of control group according to the method of random number. The patients in group A were taken papaverine orally in immediately after the surgery, group B were injected papaverine muscularly, group C were infused papaverine through vein, while the control group were given conventional treatment and did not use any vasodilator drugs. The effects of skin temperature after repairing vascular injury in limbs and it's relation with the ways of administration were explored. Results The flap survival area of group A, B, C were significantly higher than that of the control group, the differences were statistically significant (P 0.05). Conclusion Different papaverine administration ways can improve the limbs ipsilateral skin temperature after the repair of injury of limbs vascular, intramuscular injection is the best way, followed by intramuscular.
