Decreased hepatic uptake of 2-[fluorine-18] fluoro-2-deoxy-D-glucose (FDG) had been reported in the patient with hyperinsulinemia under unsatisfactory fasting and hyperthyroidism. We are presenting two cases of markedly reduced FDG avidity without abnormal increase in muscular uptake. Both patients had no history of chronic liver disease and had normal thyroid function, and well complied with the fasting protocol. Both of them experienced prolonged starvation and finally presented with hypoalbuminemia. The low avidity of liver and hypoalbuminemia could be both associated with malfunctioning hepatocytes caused by prolonged starvation.
