Once again, Scotland is leading the way. The details of the chronic medication service - to be launched in spring 2010- have now been announced (p669) and pharmacists in all parts of Britain and in all sectors should take note.Pharmaceutical care for people with long-term conditions should be every pharmacist's business - whether the condition has been diagnosed in primary care or is the result of hospital care. The larger the role pharmacists can play in ensuring that patients understand the importance of taking their medicines - what they take them for, how to take them safely and how to gain maximum benefit from them - the better patients will be and the better NHS resources will be used.
