经皮骨圆针髓内固定结合小夹板治疗肱骨干骨折临床研究

摘要

目的：观察经皮骨圆针髓内固定结合小夹板治疗肱骨干骨折的临床疗效。方法：治疗组36例采用经皮骨圆针髓内固定结合小夹板治疗，对照组48例用钢板内固定方式治疗，分析比较两组手术时间、手术出血量、骨折愈合时间、肩关节功能和并发症及不良反应情况差异。结果：治疗组在手术时间与出血量上皆明显少于对照组（P＜0.05，P＜0.01），而骨折愈合时间、肩关节功能改善比较无统计学差异。对照组骨折不愈合而内固定失效1例，治疗组术中桡神经损伤l例，对照组术中桡神经损伤2例，皆经保守治疗后神经功能完全恢复。结论：经皮骨圆针髓内固定结合小夹板治疗肱骨干骨折具有操作容易、损伤小、固定可靠、手术并发症少的特点。%Objective:To observe the clinical effect of the Percutaneous bone round needle fixation combined with the small splint to treat humeral shaft fracture.Methods:The treatment group used the Percutaneous bone round needle fixation combined with the small splint in 36 patients, while the control used the internal fixation by steel plate in 48 patients.Analyzed and compared to the differences between the two groups in operation time,the amount of operation bleeding,the fracture symphysis time,the shoulder joint function,the complication and the side effects.Results:In operation time and the amount of operation bleeding,the treatment group was significantly less than those of the control,both had obvious differences. However the difference between the fracture symphysis time and the shoulder joint function had no statistical significance.Nonunion and internal fixation having no effect emerged from one patient in the control.There was a patient appearing radial nerve injury.The control had a patient arising the radial nerve injury during the operation.They all completely recovered through conservative treatment.Conclusion:The treatment of the Percutaneous bone round needle fixation combined with the small splint had several advantages such as easier in manipulation,less lesion,reliable fixation,fewer complications and so on.
