The effectiveness of sodium bicarbonate (SB) has recently been questioned although it is often used to correct metabolic acidosis of neonates. The aim of the present study was to examine its effect on hemodynamic recovery in the resuscitation of hypoxic newborn animals with severe acidosis. Hypoxic piglets were acidotic and in cardiogenic shock. Cardiac index, stroke volume, regional blood flows and blood pressure of saline controls remained lower than baseline values during reoxygenation. SB administration significantly enhanced the recovery of both arterial pH and bicarbonate level within the first hour of reoxygenation in piglets. However, it did not cause any significant effect on temporal hemodynamic changes during reoxygenation. The marker of oxidative stress (GSSG/GSH ratio) was about the same between hypoxic groups, whereas the lactate level was slightly lower in SB-treated group.
