Introduction: High tibial osteotomy (HTO) is a widely accepted surgical treatment for unicompartmental osteoarthritis of the varus knee. Valgus overcorrec-tion of 2 to 6degrees has been shown to have optimal outcome. However, the conventional is difficult to obtain the ideal correction in terms of both accuracy and precision. Methods: Ten patients with varus knee deformities and medial unicompartmental osteoarthritis were operated with medial open wedge high tibial osteotomy (HTO) using the CT-free (BrainLAB Vector Vision VV1.0) Navigation System. Intra-operative limb alignments according to the navigation system and the post-operative XR alignments were measured. Results: The average post-operative coronal plane alignment of the limb was 2.7 degrees valgus ( range, 1 -4 degrees ). The average deviation between the intra- and post-operative alignments was 1.0 degree (range, 0.1 - 1.9 degrees ). There was no complication.
