Autoimmune hepatitis in the chronic course of HAV infection

In this paper, we present the case of a 13-year-old girl with type 1 autoimmune hepatitis in whom biochemical, serological and histopathological parameters of chronic hepatitis caused by virus A in last 2 years were observed. The diagnosis was based on increased -/-globulin levels and IgG levels and the presence of autoantibodies and was confirmed by the presence of anti-HAV IgM in serum. First, the patient was treated by steroids given by the pulse method for 5 months, and next, because improvement was not observed, continuous steroid therapy for next 5 months. After this period, clinical and biochemical remission and loss of HAV antibodies of the class IgM were observed. In the follow-up, when sustained steroid therapy was given, a clinical and biochemical exacerbation of the disease was observed, accompanied by anti-HAV IgM and insignificant intensification of the autoimmune process parameters.
