The present invention relates to a computer-implemented method for predicting a probability of an exacerbation (not leading to hospitalization) or a hospitalization of a patient due to an exacerbation of Chronic Obstructive Pulmonary Disorder, or COPD, at a prediction date. The method comprises the steps of receiving, via the one or more input elements, a set of input patient data corresponding to the patient. Further, the method comprises the step of applying a first supervised machine learning algorithm to a first historical set of patient data that includes data from a first plurality of patients having at least a COPD diagnosis at a primary or secondary care service at a first index date; based on the application of the first supervised machine learning algorithm to the first historical set of patient data, generating a first prediction model; applying the first prediction model to the set of input patient data; and finally determining and outputting the probability of a hospitalization for the patient within a first time window.
