PROBLEM TO BE SOLVED: To provide a form and heat insulation panel having high heat insulation performance, by largely improving building-up performance of a construction form.;SOLUTION: This form and heat insulation panel P for construction is composed of a panel body 1 and a binding bar 20 connected to its upper surface. In the panel body 1, an insertion hole 2 of an assembly tie 40 is arranged at an interval in its longitudinal direction on its under surface, and an easily breakable thin wall 4 for covering the insertion hole 2 is arranged on a surface on the side for contacting with it placing concrete. A plurality of first reinforcement strips 6 for indicating a position of its thin wall 4 are juxtaposed in the vertical direction in parallel.
