Parenteral Drug Treatments for Acute Migraine Headache. Technical Review 2.5

The objective was to identify and summarize evidence from controlled trials on211u001ethe efficacy and tolerability of parenteral drug treatments for acute migraine 211u001eheadache. A literature review, which included the 1996 MEDLINE database, was 211u001econducted to identify randomized controlled trials involving patients with acute 211u001emigraine headache in which at least one treatment offered was a parenterally 211u001eadministered drug treatment. The number of patients obtaining headache relief 211u001eaccording to an a priori definition of at least 50% reduction in pain severity 211u001ewas recorded and used to calculate odds ratios for headache relief. Controlled 211u001etrials provide limited support for the efficacy of a number of aprenteral 211u001etreatments, including antinauseants (prochlorperazine and metoclopramide); non-211u001esteroidal anti-inflammatory drugs, or NSAIDS (diclofenac); and opiate anagesics 211u001e(butorphanol and methadone). Weaker evidence supports the efficacy of 211u001edihydroergotamine (DHE). The principal adverse effects observed were nausea and 211u001esedation.
