This issue is the first of a two-part special issue of Perspectives in Vascular Surgery and Endovascular Therapy dedicated to vascular trauma. This issue comes at a time when standards achieved in the management of this injury pattern are being challenged. Changing practice and training paradigms along with the rapid evolution of endovascular therapies have placed excellence in vascular trauma at a crossroads. This scenario comes at a time when observations from the most prolonged military conflict in US history suggest that skills to manage vascular injury remain a requisite for deployed surgeons.
