Commentary on 'torso vascular injury'.

Drs Dente and Feliciano have once again provided us with a primer on the care of some of the most moribund and difficult-to-manage trauma patients, those with torso vascular injury. Unlike most advances in trauma care, military experience has added little to the compendium, as elucidated by the authors. Civilian wounding patterns and proximity to care in a major urban trauma center likely account for this. One might consider the hemo-static resuscitation first described and subsequently perfected by the military in the past 5 years as being a significant contribution to the management of patients with major vascular injury. Regardless, as the authors clearly cite, young trauma surgeons not well versed in penetrating injury management will find these cases highly challenging.
