Recent advances in ar-throscopic rotator cuff repair have resulted in improved outcomes. One distinct advantage of arthroscop-ic repair is visualization of the tear anatomy, especially with larger, retracted U-shaped tears. However, some studies suggest that arthroscopic techniques may be inferior to mini-open techniques for large and massive tears.Recently, there has been a trend toward using arthroscopic double-row techniques to reconstruct the rotator cuff footprint and improve fixation. The suture-bridge technique is becoming a popular option for double-row rotator cuff repair. As previously described by Park et al, medial anchors are placed first and tied. The sutures are then crossed, passed over the cuff, and placed in more lateral tuberosity anchors. When using this technique for larger tears, it can be difficult to get the lateral portion of the cuff into an anatomic position and "dog ears" are often generated. This article describes a technique that allows anatomic positioning of the cuff prior to tying the medial anchors,using a third row of fixation placed between the typical medial and lateral rows.
