We thank Drs. Natarajan, Chakravarti, and Kothari for their comments on the use of 25-gauge vitrectomy systems in the treatment of surgical retinopathy of prematurity.1 We are delighted to hear confirmation that others have concerns similar to those we specifically emphasized in the discussion section of our article. We tried to stress that careful preoperative ophthlamoscopic examination and patient selection are critical for the success in using 25-gauge systems. If during the preoperative examination, there is any uncertainty identifying optimal sites for trocar insertion, then the use of the 25-gauge transconjunctival technique was reconsidered.
