Despite the number of advantages of small incision vitrectomy, the small cutting port of the vitrector can make certain parts of the surgery more challenging than with standard 20-gauge vitrectomy.1 Induction of a posterior vitreous detachment can be especially difficult with the 25-gauge systems. A suction pick can help to effectively induce a posterior vitreous detachment. After attaching a one-and-a-half inch long 25-gauge needle to an extrusion line, the needle is bent at the distal end to form a pick.
