We report on a new shoulder orthosis to treat shoulder pain (PS) in the severely affected arm after stroke. The patient wears the orthosis directly on the skin, it consists of two pieces: one shoulder part and one forearm part. These two parts are connected with help of cross-wise adjusted straps. The offthosis rotates the forearm and supination and extends the elbow. 12 patients in the subacute phase and with a severe arm paresis were provided with the orthosis so far. They either complained about shoulder pain or they had a visible subluxation. The wearing comfort was high, and the patients did not complain about malodor. 7 of 12 patients suffered initially from PS. After the intervention period they reported a relevant reduction of shoulder pain, but what was not assessed by the rater. In three of five patients with subluxation the gap was closed after four weeks and a reduction of the gap in two patients was assessed. They did not develop a PS during the intervention period. In conclusion the orthosis is an interesting option in the treatment and prevention of shoulder pain of the severely affected arm after stroke. Further studies are needed.
