The public discussion about health care financing has an enormous relevance for the field of gynaecology and obstetrics. This paper of the financing commission of the DGGG presents essential financial aspects and problems, influencing our specialty. Regarding increasing rates of professional liability, Germany could reach a comparable situation to the USA. A reduction of the actual quality of care can be possible. Moreover, the education and training are essential foci to assure health care on a high level in future. The costs are a general problem due to doubtful representativeness of education and training costs in the DRG system. An appropriate adjustment should be demanded in the inpatient and outpatient setting. The German National Cancer Plan can have several financial influences beside effects on oncological care, e.g. regarding the early detection program of cervical cancer or the strengthening of certified centres (also important for discussions of additional charges) and improvement of tumour documentation and quality assurance. Furthermore, there is an urgent need for action to finance obstetrics and perinatal medicine. Financial resources should be increased and specific procedures of centres should be refunded by the DRG system. Solutions need multiple approaches and the collaboration of all colleagues in the field.
