I was interested to read the paper by Porpiglia et al on prostatectomy (Eur Urol 2006;49:120-6). When Terence Millin introduced his transcapsular or retropubic operation, I was a young registrar attached to Professor Charles Wells' unit at the Liverpool Royal Infirmary. He was a superb technician with a special interest in urology but he. soon gave up Millin's procedure as being too difficult and dangerous an operation for training purposes. He could, not see (nor could I) the sense in approaching the prostate through a dense venous plexus and, therefore, returned to the transvesical route.
