How Safe are our Safety Practices?

The importance of safety,health and environment in industry as well as in our daily life is very well-known.It is claimed that safety practices which are widely known are being followed.Yet isn't it amazing that in spite of our tall claims,accidents do occur.Incidents of catastrophic fires,explosions,fatal accidents are not uncommon.The reported statistics say nothing about thousands of smaller events that occur and go un-recorded.Unfortunately,usually action on these issues are taken only after some very large and tragic event occurs and not in a pro-active manner.This raises the question of what is missing in the endeavour and commitment to ensure complete safety at the work place and surroundings.Present article is an attempt to analyse the situation based on author's experience of more than 32 years in plant operation,project implementation and handling safety related matters in large fertiliser plants.
