Response to Letter to the Editor: Implementation of a Cardiac Transitions of Care Pilot Program: A Prospective Study of Inpatient and Outpatient Clinical Pharmacy Services for Patients With Heart Failure Exacerbation or Acute Myocardial Infarction

We appreciate and would like to acknowledge the additional insight and expertise shared by the individuals from Touro University California College of Pharmacy (TUCCOP) and Sharp Memorial Hospital (Sharp) into transitions of care (TOC) pharmacist programs and the positive outcomes that these pharmacist-led initiatives can have on both the health system and patient care. We were inspired after reading about the TUCCOP and Sharp TOC program and Continuum of Care Network (CCN). To the investigators' knowledge, there is not a currently existing CCN program or a similar type of support network to set up a large-scale TOC program in the state of Ohio, which is the site where the "Implementation of a cardiac transitions of care pilot program: a prospective study of inpatient and outpatient clinical pharmacy services for patients with heart failure exacerbation or acute myocardial infarction" study1 was conducted. We would like to take this opportunity to emphasize that patient care efforts were maximized through effective use of the existing resources in the institution, those that were available due to grant funding, and the multidisciplinary approach.
