The approach to coarctation of the aorta with hypoplastic aortic arch is controversial, not only regarding the definition of what is truly a "hypoplastic arch" but also for the best technique to correct it. Some of us will use a median sternotomy with car-diopulmonary support combined with circulatory arrest, others will opt for lateral thoracotomy, typically using off-pump resection and an extended end-to-end anastomosis (REEEA) that is tailored to correct the primary lesion while augmenting the arch.
