Prostate cancer is a malignancy commonly seen in geriatric male population. Many patients experience recurrence after initial radical therapy. F-18 fluorocholine PET/CT has evolved to be a new and promising image modality for detection and localization of the recurrent prostate cancerous foci. Nevertheless, the diagnostic performance in the prostate bed is quite limited, particularly because of the highly concentrated radioactivity in the urinary bladder hampers the detailed interpretation of the nearby area. Eliminating the intense radioactivity from the urinary system becomes a possible solution to improve diagnostic performance of the local recurrent prostate cancer. We herein report a case using the early post-voiding image to successfully detect a residual prostate cancer in the prostate bed on the 18F-fluorocholine PET/CT and thus conclude that the early post-voiding image is a practical method to improve the diagnostic performance of F-18 fluorocholine PET/CT and should be incorporated into the routine image protocol.
