Found in the Field — A Soldier With Heat Stroke, Exercise-Associated Hyponatremia, and Kidney Injury

Exertional heat stroke (EHS) can be fatal if not recognized and treated early with rapid cooling measures. Exercise-associated hyponatremia (EAH) also may present with similar symptoms and can easily be confused for heat stroke. Typically, EHS and EAH do not occur together. A recent report described a case of a soldier with both EHS and EAH during a timed military road march, who eventually died from his illness (1). We report another case of concomitant EHS and EAH that was successfully treated by rapid identification of the combined pathologies, aggressive field management, and use of 3% hypertonic saline.
