Fixed-bearing or mobile-bearing total knee arthroplasty? A review of the recent literature

Primary total knee arthroplasty (TKA) implants have evolved to a universally accepted configuration. However, in terms of implant selection the surgeon has some options concerning the polyethylene insert. The posterior cruciate ligament (PCL) may be retained or sacrificed and the bearing may be a fixed or mobile type. Both of these subjects have been extensively studied and the consensus of the published literature and arthroplasty registers appears to be that there is no consistent demonstrable difference in terms of clinical outcome between these designs. As far as fixed and mobile bearings are concerned there are conceptual advantages with a mobile-bearing design. In this article we examine recent and significant publications on the underlying principles and the established results.
