Dr Preiss and colleagues1 demonstrated that intensive-dose statin therapy was associated with an increased risk of incident diabetes compared with rnoderate-dose therapy in a meta-analysis of randomized trials. These results add to their prior meta-analysis that demonstrated an association between randomization to statins vs control treatment and the development of diabetes.Although the evidence is compelling, this association remains open to question. These meta-analyses are based on subgroup and post hoc analyses.3 Subgroup analyses evaluate treatment effects in a subdivision of the study population based on nonrandom baseline characteristics. Along with the lack of prespecification, this introduces analytical errors that can lead to overstated results and misleading conclusions.
