THE prescribing and dispensing of medicines is one of the most common healthcare interventions in the NHS, putting pharmacists right at the heart of patient care. However we know medicines given to patients for good reason can sometimes cause them harm. It is estimated that one in 20 hospital admissions result from an adverse drug reaction or a problem with medication.
