A locum pharmacist responsible for a series of dispensing errors has had his name struck from the Register of Pharmacists on the order of the Disciplinary Committee. The committee also admonished the pharmacy company for which the locum had been working at the time of the incidents and reprimanded its superintendent pharmacist, who had himself made dispensing errors and had failed to implement error-reporting procedures required by the Code of Ethics for Pharmacists and Pharmacy Technicians.
