Michael McCarthy's World Report (July 28, p 308) nicely summarises the success story of the Global Fund to Fight AIDS, Tuberculosis and Malaria in the past 5 years. However, we think that two important points have not received adequate attention. First, the Global Fund has signed the Paris Declaration on aid effectiveness.2 Meetings of the Global Fund leadership are necessary, but will not suffice to achieve harmonisation, which also requires attention at the country level. In many countries malaria and HIV/ AIDS control programmes are still executed in parallel, often by the same body, without even a minimum dialogue across programmes. Country coordinating mechanisms required by the Global Fund could have a more prominent role here and should be actively encouraged to pay attention to this aspect
