Postdischarge transitional care management: A reimbursable service in 2013

To the Editor: The Centers for Medicare and Medicaid Services (CMS) have recognized Transitional Care Management (TCM) as a reimbursable service in the 2013 Medicare Physician Fee Schedule.1 Effective January 2013, one of two new Current Procedural Terminology codes [99 495 or 99 496) can be submitted for the postdischarge care delivery and care coordination of an individual recently discharged from an inpatient setting (hospital, skilled nursing facility) by a qualifying healthcare provider.
