The past decade of health care has been underscored by the development of patient-centered care. Significantly, providing high-quality patient-centered care was included as a pivotal recommendation in a report by the Institute of Medicine (IOM, 2001), Crossing the Quality Chasm: A New Health System for the 21st Century. The report emphasized safe, effective, patient-centered, timely, efficient, and equitable health care (IOM, 2001). Since the publication of the IOM report, the patient-centered health home model has emerged as an exemplar of a comprehensive approach to care. Conceptually, the patient-centered health home model includes accessible, continuous, comprehensive, family centered, coordinated, compassionate, and culturally effective care (American College of Physicians, 2005).
