A 41-year-old female attended the clinic for a medical check up for insurance purposes. She was found to have a right-sided posterior mediastinal mass on chest X-ray. Subsequently the contrast enhanced CT scan showed an aneurysm beside the SVC (Fig. 1A-D). The angiogram confirmed the aneurysm at the superior venacavo-azygous junction (Fig. IE). She underwent excision of the aneurysm through a right lateral thoracotomy through the fourth intercostal space (Fig. 2A-D). During the operation the aneurysm was dissected and the azygous vein was ligated proximally. The aneurysm was clamped at the distal end close to SVC and excised.
