In January 1998 the Australian Pharmacy Advisory Council developed the National guidelines to achieve the continuum of quality use of medicines between hospital and the community (APAC Guidelines). This document was reviewed and expanded in 2005 as Guiding principles to achieve continuity of medication management, now more generally addressing the problems in medication management as health consumers move from one episode of healthcare to another.
