Laparoscopic Lumbar Spinal Fusion Using the BAK Interbody Fusion Device

Minimally invasive techniques for the treatment of intervertebral disc disease have been developed. The technique for the laparoscopic insertion of the BAK, a threaded lumbar interbody fusion device, is described, Using the BAK device, 831 patients underwent anterior lumbar interbody fusion; the procedure was performed laparoscopically in 265 patients and through a traditional retioperitoneal approach in 591. In single level surgery (L4-L5 or L5-S1), fusion was obtained in 91.4% and 92.1% of the laparoscopic and open procedures, respectively. For two level surgery (L4-L5 and L5-S1), fusion was obtained in 91.7% and 78.7% of the laparoscopic and open procedures, respectively. Hospital length of stay was shorter in the laparoscopic group for single level cases (3.3 versus 4.0 days); however, this trend was reversed for the two level cases. From the data, we conclude that the BAK device inserted laparoscopically can achieve solid fusion with a minimum morbidity.
