Cystectomy to treat muscle-invasive bladder cancer has become a common procedure in current urologic practice. This has led to great improvements in surgical, anesthetic, and postoperative care applications in such patients that have reduced the morbidity and mortality rates even in elderly. However, the numbers are still considerable. In the present study, Liberman et al report a 2.8-fold and 5.0-fold greater 90-day postoperative mortality rate in septuagenarians and octogenarians, respectively, compared to their younger counterparts. However, we believe 2 points, although referred to in Comment section, need to be addressed further.
