LES CRITÈRES D'ADÉQUATION SONT-ILS BIEN ADÉQUATS?

L'évaluation de l'adéquation («appropriateness») de procédures médicales dans diverses situations cliniques nécessite la définition de critères d'adéquation, qui sont le plus souvent établis par des panels d'experts selon la méthode de la RAND Corporation. Toutefois, la repro-ductibilité des critères d'adéquation établis par des panels différents est souvent médiocre. En prenant l'exemple du diagnostic de la thrombose veineuse profonde, pour lequel il existe plusieurs algorithmes diagnostiques concurrents, cet article tente d'analyser les facteurs qui expliquent cette variabilité entre experts. En l'état actuel, cette variabilité est trop importante pour rendre les critères d'adéquation contraignants pour le médecin. Ils peuvent toutefois servir à informer sa décision dans certains domaines.%The assessment of the appropriateness of medical interventions in specific clinical situations rests on the definition of appropriateness criteria. These criteria are usually established by expert panels according to the RAND Corporation method. However, numerous studies raise concern on the reproductibility of these criteria. Through the example of the diagnosis of deep venous thrombosis and its various algorithms endorsed by different experts, this paper attempts to analyze the factors which explain this variability. Presently, this variability is too great to allow the use of appropriateness criteria to enforce medical decision making. However, they may be useful to inform the physician's decision in certain situations.
