On 2 May 2007 a Chief Engineer Officer with more than 10 years experience as Chief Engineer lost his life in an oily bilge tank. At 08.20 that day he had instructed a wiper to open the tank manhole. He did not say why he wanted it opened and did not request any further assistance from the engine room staff who were busy performing other tasks. Nobody apart from the wiper was aware that he had asked for the tank to be opened.
