Background: The ageing populations are growing with an increased demand to improve quality of aged care facilities and increased activity participation is fundamental for improving cognitive and overall health of older people. This study aimed to identify predictors for physical and leisure activities among older people in Swedish aged care facilities. Methods: Baseline data from Swedish National Inventory of Health and Care in Nursing Homes (U-Age Swenis) on participation in physical and leisure activities were analyzed relating to symptoms of cognitive impairment, pain, dependence in activities of daily living and health status. Results: Data on 4,831 residents showed mean age of 85.5 years, 67% females and mean length of stay was 30.5 months. Half of them participated in physical activities and 92% participated in at least one leisure activity during the week preceding data collection. Participation in at least one leisure activity (adjusted OR 5.50, 95% CIs 3.27–9.25) and living in dementia specific unit (adjusted OR 1.86, 95% CIs 1.54–2.25) were strong predictors for physical activities. Being dependent in ADL (adjusted OR 0.59, 95% CIs 0.50–0.71), showing symptoms of extreme anxiety/depression (adjusted OR 0.64, 95% CIs 0.48–0.87) and severe cognitive impairment (adjusted OR 0.51, 95% CIs 0.38–0.69) predicted less participation in physical activities. Being aged >85 years (adjusted OR 0.47, 95% CIs 0.31–0.72) and having symptoms of cognitive impairment predicted less likelihood of participation in leisure activities. Conclusions: Identified predictors can be used to develop targeted strategies for increasing physical and leisure activities among older people in aged care facilities.
