冠心病患者外周血单核细胞中EMMPRIN、MMP-2表达变化及与冠状动脉粥样硬化的关系

         

摘要

目的 观察冠心病患者外周血单核细胞中细胞外基质金属蛋白酶诱导因子( EMMPRIN)、基质金属蛋白酶2(MMP-2)表达变化,并探讨其与冠状动脉粥样硬化的关系.方法 153例冠心病患者,其中稳定型心绞痛86例( SAP组)、急性冠状动脉综合征67例( ACS组). 同期健康体检者37 例为正常对照组( NC组) ,采用Western blotting法及qRT-PCR法检测三组外周血单核细胞中EMMPRIN、MMP-2蛋白及mRNA表达,对EMMPRIN mRNA、MMP-2 mRNA与粥样硬化斑块分型、冠脉造影粥样斑块病变分型进行分析. 结果 EMMPRIN、MMP-2在ACS组及SAP组蛋白及mRNA水平均高于NC组(P均<0.05). EMMPRIN mRNA、MMP-2 mRNA在ACS及SAP组中表达与粥样硬化斑块分型、冠脉造影粥样斑块病变分型呈正相关(r分别为0.341、0.391、0.275、0.142, P均<0.05).ACS组及SAP组中EMMPRIN mRNA、MMP-2在软质斑块及Ⅱ型表达量较高. 结论 EMMPRIN及MMP-2在冠心病患者外周血单核细胞中表达上调,二者与冠状动脉粥样硬化的发生、发展有关.%Objective To observe the extracellular matrix metalloproteinase inducer ( EMMPRIN) and matrix metallo-proteinase 2 ( MMP-2) expression changes in the peripheral blood monocytes of patients with coronary heart disease , and to discuss their correlations with coronary arteriosclerosis .Methods A total of 153 patients diagnosed as coronary heart dis-ease were divided into the stable angina pectoris group (SAP group, 86 patients) and the acute coronary syndrome group ( ACS group , 67 patients ) , and another 37 normal healthy volunteers were selected as the normal control group ( NC group) .The EMMPRIN and MMP-2 expression at protein and mRNA levels in the peripheral blood monocytes of the three groups were detected by Western blotting and qRT-PCR.The correlations of EMMPRIN mRNA and MMP-2 mRNA with ar-teriosclerosis plaque grade and coronary angiography types were analyzed .Results EMMPRIN and MMP-2 expression in the ACS group and SAP group was higher than that of the NC group at protein and mRNA levels (all P<0.05).The EMMPRIN mRNA and MMP-2 mRNA expression in the ACS group and SAP group was correlated with arteriosclerosis plaque grade and coronary angiography types (r=0.341, 0.391, 0.275 and 0.142, respectively; all P<0.05).The EMMPRIN mRNA and MMP-2 mRNA expression was higher in the soft plaque and typeⅡpathological changes of the ACS group and SAP group .Conclusions The EMMRPIN and MMP-2 expression in the peripheral blood monocytes is up-regu-lated in patients with coronary heart disease , and they were correlated with the occurrence and development of coronary ar-teriosclerosis.
