肺炎链球菌流行基因型的细菌毒力及大环内酯类的耐药机制

         

摘要

Objective To study the relationship between the drug resistance mechanism of macrolides and bacterial virulence in inva-sive pneumococcal disease (IPD) with epidemic genotypes .Methods A total of 12 strains of IPD from June 2013 to June 2016 were collected as observation group ,and other 10 strains of carrying Streptococcus pneumonia (S .pn) were used as control group .Then the serum genotypes were compared by multiplex PCR amplification method ,and the mutation of base sequences of virulence genes including cps2A ,bacteriocin ,psa A ,pcs B ,ply and clp P were analyzed by PCR .The drug resistance rates and minimum inhibitory concentrations (MIC) of penicillin ,cefepime ,levofloxacin ,erythromycin and meropenem were determined by MIC method .Results The rates of 14 ,6A/6B and 19F genotypes in the two groups showed no statistical difference (P>0 .05) ,while the 19A ,6C and 17F in observation group were significantly higher (P0 .05) ,while the sensitive rates and MIC values of the others showed no statistical difference (P0.05),19A、6C和17F在观察组中检出率显著高于对照组(P0.05).观察组对青霉素和红霉素不敏感率和MIC值显著高于对照组(P<0.05).结论 分离的IPD血清型主要有19A、6C和17F,共有6种致病毒力基因,对青霉素和红霉素耐药.
