黄芎抗栓胶囊治疗伴应激性高血糖的老年急性缺血性脑卒中患者效果观察

ObjectiVe It is to observe the effect of Huangxiong Kangshuan capsule on serum inflammatory factors,insulin resistance and recent recovery of neurological function in old acute ischemic stroke patient with stress hyperglycemia. Methods 107 old acute ischemic stroke patient with stress hyperglycemia were randomly divided into observation group( n = 54)and control group(n = 53). The control group was given normal western medicine,while the observation group was added with Huangxiong Kangshuan capsule on the basis of treatment in control group. Both treatment courses were 14 days. The changes of serum hs - CRP,IL - 6 and TNF - α,HOMA - IR,2hPG,FBG,CSS scores and BI scores before treatment and in 7,14 days after treatment were observed in both groups,and the curative effects were compared. Results The levels of serum hs -CRP,IL - 6 and TNF - α,HOMA - IR,2hPG,FBG were significantly decreased after treatment compared with than before treatment in both groups,and the decrease in observation group were more significant( P < 0. 05). In 7,14 days after treat-ment,CSS scores were obviously decreased while BI scores were increased in both groups,and the improvements in observa-tion group were better than that in control group(P < 0. 05). The total effective rate of observation group was higher than that in control group(P < 0. 05). No serious side effect was found during treatment in both groups. Conclusion Huangxiong Kang-shuan capsule can significantly decrease the levels of serum inflammatory factors,improve insulin resistance and regulate blood sugar,improve neurological function and life quality in old acute ischemic stroke patient with stress hyperglycemia.%目的：观察黄芎抗栓胶囊对伴有应激性高血糖的老年急性缺血性脑卒中患者血清炎性因子、胰岛素抵抗及近期神经功能恢复的影响。方法将107例伴有应激性高血糖的老年急性缺血性脑卒中患者随机分为观察组（54例）和对照组（53例），对照组给予常规西医治疗，观察组在对照组基础上接受黄芎抗栓胶囊治疗，疗程均为14 d。观察2组治疗前及治疗7 d、14 d 后血清炎性因子超敏 C 反应蛋白（hs - CRP）、白细胞介素-6（IL -6）及肿瘤坏死因子-α（TNF -α）水平，胰岛素抵抗指数（ HOMA - IR）、餐后2 h 血糖（2hPG）、空腹血糖（FBG），CSS 评分及 BI 评分变化情况，并统计2组临床疗效。结果2组治疗7 d、14 d 后血清 hs - CRP 、IL -6、TNF -α、HOMA - IR、2hPG、FBG 均较治疗前明显降低（ P 均<0.05），且观察组上述指标水平在以上时间点均明显低于对照组（P 均<0.05）；观察组治疗7 d、14 d 后 CSS 评分均明显降低（ P均<0.05），BI 评分均明显升高（P 均<0.05），且观察组上述评分改善情况均明显优于对照组（ P 均<0.05）；观察组临床总有效率高于对照组（P <0.05）；2组治疗期间均无严重不良反应发生。结论黄芎抗栓胶囊能够显著降低伴应激性高血糖的老年急性缺血性脑卒中患者血清炎性因子水平，改善胰岛素抵抗并调节血糖水平，促进患者神经缺损功能的恢复，提高日常生活活动能力。
