承德市医患关系现状及影响因素分析

摘要

目的：了解承德市医患关系现状，并提出针对性对策，为改善医患关系提供参考。方法：采用方便抽样的方法，选取承德市3所医院的1047名医护人员和506名住院患者及家属进行问卷调查。结果：对医患关系现状的总体评价，医方认为和谐的占24．9％，患方认为和谐的占55．5％，两者比较差异有统计学意义（ P＜0．01）。影响医患关系紧张的3个维度中，患方认为环境因素最重要，医方认为患者因素最重要。其中政府对医疗机构投入不足、医药体制不合理、医患沟通少、个别医护人员出现医疗缺陷、患者对医护人员的工作缺乏基本的理解和尊重、患者对治疗期望过高等是各个维度中影响医患关系的主要因素。结论：承德市医患关系现状总体较好，但存在需改进的空间。需要强化政府职责，推进医疗卫生体制改革；加强医疗机构内部管理，提高医务人员的素质和服务水平；加强全民医疗健康知识教育，让患者了解医疗服务的特殊性，理解和尊重医学科学。%Objective: To understand the current status of the doctor-patient relationship in Chengde, and to put forward corresponding so-lutions to provide references for improving the doctor-patient relationship.Methods:With method of convenience sampling, 1047 medical staff and 506 patients and their family members were selected from 3 hospitals in Chengde to conduct a questionnaire survey.Results:With regard to the overall evaluation of status of the doctor-patient relationship, medical staff believed that the harmonious accounted for 24.9%, while the pa-tient believed that the harmonious accounted for 55.5%, and the difference was statistically significant (P<0.01).Among three dimensions af-fecting the tense relationship between doctors and patients, patients believed that the environmental factor was the most important, while medical staff believed that the patient's factor was the most important.The lack of government investment in medical institutions, unreasonable medical sys-tem , a lack of communication between medical staff and patients, medical defects caused by some medical staff, a lack of basic understanding and respect to medical staff, and patients'high expectations of treatment were the major factors influencing the doctor-patient relationship in the three dimensions.Conclusion:To establish a good relationship between medical staff and patients, it's needed to strengthen the responsibility of govern-ment, to promote the reform of the medical and health system, strengthen the internal management of medical institutions, and improve the quality of medical workers and service levels.We should also strengthen the public health education so that patients are able to understand the particulari-ty of medical service, and respect the science of medicine.
