三维超声斑点追踪技术对原发性高血压患者左心室收缩功能的评价作用

         

摘要

目的:探讨实时三维斑点追踪成像技术对原发性高血压患者左心室收缩功能的评价作用。方法:将72例患者分为左心室正常构型(LVN)组38例,左心室重构致左心室肥厚(LVH)组34例,选择同期健康体检者38例为对照组,对三组LVESV、LVEDV、LVEF、LVMI,以及左心室三维整体应变参数进行比较。结果:三组LVEF比较差异均无统计学意义(P>0.05);与LVN组、对照组相比,LVH组LVESV、LVEDV、LVMI增高,LVGLS、LVGCS、LVGRS、LVGAS明显减低,组间比较差异均有统计学意义(P<0.05)。LVN组LVGLS、LVGCS均低于对照组(P<0.05),LVGRS、LVGAS与对照组比较差异均无统计学意义(P>0.05)。结论:RT 3D-STI能准确评价心肌整体及局部功能变化,早期发现原发性高血压患者左心室收缩功能的改变,对临床治疗及疗效评价有指导作用。%Objective:To explore the effect of evaluation on three-dimensional ultrasound speckle tracking technology on left ventricular systolic function of the patients with primary hypertension.Method:72 patients were divided into the left ventricular normal geometric (LVN) group of 38 cases and the left ventricular hypertrophy (LVH) group of 34 cases which induced by ventricular remodeling.Then took the 38 healthy persons at the corresponding period as the control group.The LVESV、LVEDV、LVEF、LVMI and the total three-dimensional strain parameters of the left ventricle of three groups were compared.Result:There was no significant difference among the three groups on LVEF scores(P>0.05);compared with LVN group and control group, the LVESV、LVEDV and LVMI scores of the LVH group raised,the LVGLS、LVGCS、LVGRS and LVGAS scores of the LVH group were lower,there were significant differences among the three groups (P<0.05).The LVGLS and LVGCS scores of the LVN group were lower than those of the control group(P<0.05),the LVGRS and LVGAS scores of the LVN group had no significant differences compared with control group(P>0.05).Conclusion:The RT 3D-STI can evaluate the change of the total and partial function of the myocardium,discover the change of the left ventricular systolic function of the patients with primary hypertension earlier,it has the guiding function to clinic treatment and therapeutic evaluation.
