新生儿乙肝疫苗接种后无(弱)应答的影响因素研究

摘要

Objective To investigate the influencing factors of non-and low-response to hepatitis B vaccina-tion in infants.Methods 334 infants born during 2012-2014 were vaccinated with 3 dosage of hepatitis B vaccine according to schedule were selected as subjects.Anti-HBs were detected 2 months later,and related information of gestation period,birth,immunization,postnatal development were collected by questionnaires.Results Logistic regression analysis shows that history of maternal HB (OR = 2.045, 95% CI :1.008-6.356)and postnatal development (OR =5.001,95% CI :1.079-7.416)were the factors of non-and low-response to hepatitis B vaccination in infants.Conclusion This study shows that non-and low-response to hepatitis B vaccination in infants were caused by a variety of factors.Mothers with a histo-ry of hepatitis B infection should be careful during pregnancy,child delivery,child feeding and pay more attention to newborn nutrition.%目的：探讨新生儿乙肝疫苗接种后出现无（弱）应答的影响因素。方法选择2012－2014年出生的新生儿334例，按免疫程序接种3针乙肝疫苗，2个月后检测 Anti-HBs，问卷调查了解孕期、婴儿出生、疫苗接种及出生后发育情况，分析出现无（弱）应答的影响因素。结果Logistic 回归分析显示，与新生儿乙肝疫苗无（弱）应答有关联的因素有母亲乙肝史（OR ＝2．045，95％ CI ：1．008～6．356）、新生儿发育状况（OR ＝5．001，95％ CI ：1．079～7．416）。结论新生儿乙肝疫苗接种后出现无（弱）应答是由多种因素导致的。有乙肝感染史的母亲应慎重妊娠、分娩及喂养孩子，新生儿要注意营养，确保正常发育。
