临床护理路径在门诊血液透析患者中的应用研究

摘要

目的：探讨临床护理路径在门诊尿毒症透析患者中的应用效果。方法对重庆市黔江中心医院收治的82例患者采用前后对照研究，2012年11月至2013年10月采用传统护理方法，作为对照组，2013年11月至2014年10月采用临床护理路径，作为观察组，对比分析两组患者依从性达标、心理焦虑状态、满意度、非计划再入院情况、健康教育知晓情况等。结果观察组患者心理焦虑发生率[73.17%（60/82）]明显低于对照组[90.24%（74/82）]，非计划再入院率[9.75%（8/82）]明显低于对照组[26.83%（22/82）]，就医依从性达标率[87.80%（72/82）]高于对照组[70.73%（58/82）]，对护理人员的护理满意度[95.12%（78/82）]明显高于对照组[74.39%（61/82）]，健康教育知晓率[92.68%（76/82）]明显高于对照组[70.73%（58/82）]，差异均有统计学意义（P<0.05）。结论临床护理路径能提高门诊血液透析患者的就医依从性和健康教育知晓率，改善焦虑状态，减少非计划再入院的发生，提高患者对护理服务的满意度，是一种促进门诊血液透析护理质量和患者自我管理的有效方法，值得推广。%Objective To investigate the application effect of clinical nursing pathway in outpatient dialysis uremic pa-tients. Methods Eighty-two patients treated in Qianjiang Central Hospital of Chongqing ,adopted the before-after control study. The traditional nursing method was adopted from November 2012 to October 2013 ,which served as the control group. The clinical nursing pathway was adopted from November 2013 to October 2014 ,which served as the observation group. The reaching standard of patient compliance,psychological anxiety status,satisfaction,unplanned readmission situation and health education awareness were contrastively analyzed between the two groups. Results The psychological anxiety occurrence rate in the observation group was significantly lower than that in the control group[73.17%(60/82) vs. 90.24%(74/82)],unplanned readmission rate was signifi-cantly lower than that in the control group [9.75%(8/82) vs.26.82%(22/82),P<0.05];reaching standard rate of compliance in the observation group was higher than that in the control group [87.80%(72/82) vs. 70.73%(58/82)],satisfaction on nursing staff in the observation group was significantly higher than that in the control group[95.12%(78/82) vs. 74.39%(61/82),P<0.05];the health education awareness rate in the observation group was significantly higher than that in the control group[92.68%(76/82) vs. 70.73%(58/82),P<0.05]. Conclusion The clinical nursing pathway can increase the patient compliance and health education awareness rate, improves anxiety status, reduces the unplanned readmission rate and elevates the patient′s satisfaction on nursing service,which is an effective method to promote the nursing quality of outpatient hemodialysis and patient self-management, and deserves to be popularized.
