三种手术方式治疗青光眼并白内障临床疗效比较

摘要

目的：：对比分析三种不同手术方式用于治疗青光眼伴白内障的疗效。方法：抽取2012年1月—2014年12月在我院就诊的青光眼伴白内障患者128例,以随机数字表法分为 A 组(42例)、B 组(60例)和 C 组(26例)。A 组行抗青光眼术联合后期白内障手术两阶段手术,B 组行白内障超声乳化术+人工晶状体植入术二联手术,C 组在 B 组的基础上行小梁切除术,对比三组的临床治疗效果。结果：三组术后眼压均显著降低,且 C 组显著低于 A、B 组(P <0.05)；术后视力均显著提高,且 C 组显著高于 A、B 组(P <0.05)；三组并发症发生率无明显差异(P >0.05)。结论：两期手术、白内障超声乳化术联合人工晶状体植入术以及行白内障超声乳化吸出术联合小梁切除术治疗青光眼伴白内障均具有较好的疗效,以后者疗效最佳,临床应根据患者的具体情况合理选择手术方式。%Objective:Comparative analysis of three different modes of operation for the therapeutic effect in the treat-ment of glaucoma with cataract.Methods:Selected from January 2012 to December 2014,glaucoma in our hospital for treatment of 128 cases of patients with cataract,to randomly divided into A group (42 cases),B group (60 cases)and C group (26 cases).The A group performed anti glaucoma surgery combined with cataract operation in late two stage operation,group B underwent phacoemulsification and intraocular lens implantation combined + two operation,group C in the B group on the basis of trabeculectomy,and compared the clinical efficacy of the three groups.Results:The three groups of postoperative intraocular pressure decreased significantly,and the C group was significantly lower than A,B group(P <0.05);postoperative visual acuity was improved significantly,and the C group was significantly higher than A,B group(P <0.05);the three groups the incidence of complications had no significant difference (P >0.05).Con-clusion:Two surgery,phacoemulsification combined with intraocular lens implantation,phacoemulsification combined with trabeculectomy for treatment of glaucoma complicated with cataract were with good effect,the latter best effect, clinical should according to the specific condition of patients and reasonable choice of surgical approach.
