目的 分析声像图中含微钙化灶的乳腺癌不同病理类型之间的关系.方法 经手术或穿刺活检证实的116例乳腺癌患者,将不同病理类型肿块的微钙化灶发生率进行相关性分析,并选择肿瘤的大小、边界、形态、边缘、后方回声及血流信号等6项反映肿瘤生物学行为的指标,对含微钙化灶的不同病理类型的乳腺癌进行分析.结果 浸润性导管癌和导管内癌的徽钙化灶发生率远高于其他类型,差异有统计学意义(P＜0.05),导管内癌的微钙化灶发生率略高于浸润性导管癌,但差异无统计学意义(P＞0.05);含微钙化灶的浸润性导管癌与导管内癌在肿瘤的边缘是否呈蟹足状、后方回声有无衰减方面,差异均有统计学意义(P＜0.05),但在大小、边界、形态及血流信号分级方面,差异均无统计学意义(P＞0.05).结论 声像图中含微钙化灶的乳腺癌多见于导管癌(包括导管内癌和浸润性导管癌),结合肿块的其他声像图表现,对肿块的病理组织学类型有提示作用.%Objective To analyze the relationship between the ultrasonograms microcalcification and different pathologic types of breast cancer. Methods One hundred and sixteen patients with breast cancer confirmed by surgery or biopsy were enrolled in this study. The microcalcification proportion in different pathological types of tumor was correlatively analyzed.The tumor size, boundary , shape, margin , posterior echo and blood flow signals were measured,then the microcalcification with different pathological types of breast cancer were correlatively analyzed. Results The microcalcification occurence rate in invasive ductal carcinoma and intraductal carcinoma was higher than that in other types of carcinoma, there was significant difference( P ＜ 0. 05 ) , while the calcification occurence rate in intraductal carcinoma was slightly higher than that in invasive ductal carcinoma, there was no significant difference ( P ＞0. 05) . There was significant difference of Crab - like edge, posterior echo attenuation between invasive ductal carcinoma and intraductal carcinoma ( P ＜ 0. 05 ) , while there was no significant difference of size, boundary , shape and blood flow signals ( P ＞ 0. 05 ) . Conclusion Sonographic microcalcification is more common in ductal carcinoma (including ductal carcmoma in situ and invasive ductal carcinoma) , combined with other ultrasonographic appearances of tumors , it can provide important information for diagnosis of histopathological type of tumors.
