自体骨-腱-骨移植物与LARS人工韧带重建前交叉韧带的比较

         

摘要

目的 比较自体骨-腱-骨(BPTB)和LARS人工韧带重建前交叉韧带(ACL)的效果.方法 对59例ACL断裂患者采用自体BPTB重建ACL 29例(BPTB组),采用LARS人工韧带重建ACL 30例(LARS组).按Lysholm、Tegner、IKDC评分系统和KT-1000检查评估功能.结果 59例均获随访,时间30~36个月.术后BPTB组和LARS组Lysholm评分分别为92.90分±9.10分和94.00分±6.87分;Tegner 评分分别为5.76分±1.12分和6.07分±1.14分;KT-1000检查松弛分别为2.63 mm±2.16 mm和2.36 mm±2.08 mm.术后IKDC评分:BPTB组正常14例,接近正常11例,不正常4例;LARS组正常18例,接近正常8例,不正常4例.两组各项结果 比较差异无统计学意义(P>0.05).结论 自体BPTB和LARS人工韧带重建ACL均能够获得满意的临床效果.%Objective To compare the outcome of anterior cruciate ligament ( ACL Reconstruction using bone-patellar tendon-bone( BPTB ) autografts and LARS ligaments. Methods The ACL reconstruction using BPTB autografts in 29 patients and LARS ligaments in 30 patients were performed. All patients were evaluated using the Lysholm knee score, Tegner score, International Knee Documentation Committee ( IKDC ) score, and KT-1000 arthrometer test. Results 59 patients were followed up from 30 -36 months. In the BPTB group postoperatively, Lysholm score was 92. 90 ±9. 10, and Tegner score was 5. 76 ± 1. 12. 14 cases were normal, 11 cases were nearly normal and 4 cases were abnormal according to IKDC score. In the LARS group postoperatively, Lysholm score was 94. 00 ±6. 87, and Tegner score was 6. 07 ± 1. 14. 18 cases were normal, 8 cases were nearly normal and 4 cases were abnormal according to IKDC score. The KT-1000 examination showed the mean side-to-side difference in anterior translation was 2. 63 mm ± 2. 16 mm in the BPTB group and 2. 36 mm ± 2. 08 mm in the LARS group. There were no significant differences between the two groups with respect to those scores. Conclusions Our study demonstrates that the similarly good clinical results are obtained after ACL reconstruction using BPTB autografts or LARS ligaments.
