骨桥蛋白和组织多肽特异性抗原在胃癌发生发展中的变化及临床意义

摘要

Objective To explore the change and clinical significance of serum osteopontin( OPN)and tissue polypeptide specific anti-gen( TPS)in the development of gastric cancer. Methods Thirty cases of benign gastric disease( benign gastric disease group),30 cases of gas-tric cancer( gastric cancer group),and 40 healthy subjects( control group)who were treated during June 2008 to June 2013 were included in this study. OPN and TPS were detected with ELⅠSA. The levels of OPN and TPS were compared among the three groups. Results Compared with the control group and benign gastric disease group,gastric cancer patients had significantly higher serum levels and positive rates of OPN and TPS,the differences were statistically significant( P <0. 05). OPN and TPS levels were higher in patients with well differentiated and mederately differen-tiated gastric cancer than those with poorly differentiated stomach cancer( P <0. 05);and they were higher in patients with lymph node metastasis than those without lymph node metastasis( P <0. 05). OPN and TPS levels were higher in clinical stage Ⅲ/Ⅳthan stageⅠ/Ⅱ( P <0. 05). By spearman rank correlation analysis,serum OPN expression was positively correlated with TPS expression( r =0. 363,P <0. 01). Conclusion OPN and TPS may indicate the occurrence and development of gastric cancer,and associate with the differentiation,lymph node metastasis and clinical stage of gastric cancer.%目的：探讨血清骨桥蛋白（ OPN）、组织多肽特异性抗原（ TPS）在胃癌发生发展中的变化及临床意义。方法选取2008年6月至2013年6月收治的60例患者，其中胃良性病组30例，胃癌组患者30例；另选健康受检者40例作为对照组。采用ELⅠSA方法对三组的血清OPN、TPS进行检测，并进行比较分析。结果与对照组和胃良性病组比较，胃癌组患者血清OPN、TPS水平均有显著升高，阳性率均有明显升高，差异具有统计学意义（ P <0.05）；与低分化胃癌患者相比，高中分化的胃癌患者血清中OPN和TPS的含量较高（ P <0.05）；与无淋巴结转移患者相比，伴有淋巴结转移的患者血清中OPN和TPS的含量较高（ P <0.05）；与Ⅰ/Ⅱ期患者相比，胃癌临床分期Ⅲ/Ⅳ期患者血清中OPN和TPS的含量较高（ P <0.05）；Spearman等级相关分析，血清中OPN表达与TPS表达呈正相关（ r =0.363，P <0.01）。结论 OPN和TPS在胃癌的发生发展中有一定指示作用，与胃癌分化程度、淋巴结转移情况及临床分期存在一定的内在联系性，共同促进胃癌的发生发展。
